Payslips

Separated employees requesting a payslip and/or tax form from April 2015 and forward, can access their payslip and/or tax forms ONLY in Workday for a period of 60 days after their separation date.

Click here to request replacement payslips and/or tax forms prior to April 2015 or if it has been more than 60 days after the separation date.

Workday Access

Employees who separate have 60 days from their separation date to log into Workday and download ONLY  payslips and tax documents (all other access will be deactivated). If you have trouble logging in to Workday within those 60 days, please try the steps below:

1. use a private or incognito tab in your browser, or
2. clear ALL browsing data (cookies, cache, downloads, etc.)
3. restart your browser

Form 1095-C, Employer-Provided Health Insurance Offer and Coverage

The University is required to send Form 1095-C as part of the Patient Protection and Affordable Care Act to all employees who are eligible for University medical benefits.

The vendor will mail Form 1095-C to the home address on file in Workday by March 2. A paperless option is not available for separated employees. Note, this form is not required to complete your income taxes.

Change of Address

To ensure you receive important information from the University, if you have an address change after your separation, please send a clear, color copy of your updated state identification reflecting the new address, to BenefitsExpert@miami.edu.

Your address will be changed in Workday and will be transmitted to any applicable vendors.
